Affinity Golf Program:

Submission Requirements

  • Golf and Country Club Supplemental Application as well as a Tank Pollution or Environmental Application must be completed and signed by the insured.

  • Expiring, plus 5 years prior hard copy loss runs.
    LOSS RUNS MUST BE CURRENTLY VALUED.


  • Completed Acord Applications - Brokers only.

  • Schedule of Vehicles and a Complete Drivers List, including any family members that may have use of a vehicle. The operator of each private passenger vehicle must be specified.

  • Completed and Signed Directors and Officers/Employment Practices and Fiduciary Liability Application.

  • Current, Audited Financials and completed Business Income Worksheet. If D&O coverage is requested please include a Copy of the Club By-Laws, Employee Handbook, Current 5500 and a Schedule of Directors and Officers.

  • Complete schedule of Golf Course Equipment and Golf Carts at Replacement Cost Values.

  • Signed Statement of Values including Construction, Occupancy, Year Built/Remodeled and Square footage.

  • Current Declarations page for the following: Employee Benefits Liability, Pesticide/Herbicide Liability and Tank Pollution or Environmental Policies if a retro date is requested.

  • Copies of Pesticide/Herbicide Applicator Licenses.

  • Expiring Premiums as well as Copies of all Current Policies Declaration Pages.
